What type of clinic are you?
We are a mobile primary care clinic that brings healthcare directly to patients whether at home, in assisted living, or in long-term care facilities.
Where do you provide services?
We serve multiple locations across California.
East Bay (Alameda & Costa Contra Counties) – Oakland, Berkeley, Hayward, Castro Valley, Walnut Creek, San Leandro, Union City, Fremont.
Manteca & Central Valley – Manteca, Lathrop, Stockton, Ripon
San Jose & South Bay – San Jose, Campbell, Santa Clara, Milpitas.
Northern California (Extended Coverage) – Case-by-case visits for rural or smaller towns.
Southern California – Telehealth services & select partner clinic visits.
What services do you offer?
We specialize in primary care and internal medicine for adult and geriatric patients. Services include home visits, chronic disease management, sick visits, medication management, and more.
Do you offer telehealth appointments?
Yes, we offer telehealth consultations when appropriate, based on medical needs and patient preference. Just fill out the appointment request form and our team will contact you.
How do I schedule an appointment?

Do I need a referral?
Not always. While some insurances or specialists may require a referral, you can also contact us directly for primary care services.
