  • • East Bay (Alameda & Costa Contra Counties) – Oakland, Berkeley, Hayward, Castro Valley, Walnut Creek, San Leandro, Union City, Fremont. 

    • Manteca & Central Valley – Manteca, Lathrop, Stockton, Ripon  

    • San Jose & South Bay  – San Jose, Campbell, Santa Clara, Milpitas.

    • Northern California (Extended Coverage)  – Case-by-case visits for rural or smaller towns.

    • Southern California – Telehealth services & select partner clinic visits.

How do I schedule an appointment?

Do I need a referral?

• Not always. While some insurances or specialists may require a referral, you can also contact us directly for primary care services.

Is there a co-pay for visits?

• Co-pays depend on your insurance plan. We’ll check eligibility and inform you before the appointment.
