Our Referral Process

To refer a patient, please complete the referral form below and include any recent and relevant medical records, such as lab results or imaging reports.

Once we receive your referral, our team will handle the rest:

• Schedule an initial medical consultation to assess the patient's current needs and care goals.

• If appropriate, we may coordinate with other specialists to ensure a well-rounded plan of care.

• Assist with insurance authorization and submit necessary claims on behalf of the patient.

• Our NP/Physician will conduct regular follow-ups, monitor health outcomes, and adjust care as needed.

• Maintain communication with referring providers to share updates and ensure continuity of care.

• When specialized care or long-term management is needed outside our scope, we will coordinate a smooth transition back to you or the appropriate provider.
